Resolution
Please ensure you are on the latest IRIS version first – Help and About. If you are not on latest version, then update and regenerate the return, however we would also recommend you contact HMRC at the same time whilst you start the process to update, as it is usually an issue from their side, see below:
HMRC have confirmed that the 3001 5093 error message you are receiving could be down to the clients UTR being incorrect. Please contact HMRC and ask to speak to a senior, then ask them to check whether they have the correct information on their servers that correspond with IRIS
